What If Your Next SIBO Flare Needs a Better Question, Not Another Protocol?

There is a moment I recognize immediately when I’m talking with a woman who has been dealing with #SIBO (small intestinal bacterial overgrowth) for a long time.

Something starts feeling off again. The bloating creeps back in, bowel movements change, a few foods suddenly feel questionable, and within about twelve minutes her brain has reopened the entire case file. Do I need another breath test? Should I restart the antimicrobials? Was it the probiotic? Do I need to go low FODMAP again? Wait, what did that practitioner say about biofilms three years ago?

And suddenly she is back at work as the unpaid, slightly exhausted CEO of her own gastrointestinal investigation. EEEK LOL.

I understand exactly WHY we do this. When you have had SIBO before and something worked, it makes complete sense to reach for it again. I am not anti-treatment, anti-testing, or anti-protocol. There are absolutely times when we need targeted antimicrobial support, motility support, digestive support, food changes, supplements, testing, or all of the above.

What interests me is what happens when we keep returning to essentially the same conversation without stopping to ask why we keep ending up here. That is when my brain starts getting nosy.

If someone tells me, “My SIBO came back,” I want to know what came back actually means. What happened in the months before that? Did constipation quietly return first? Did sleep fall apart or was there a huge stress season, travel, illness, hormonal shift, medication change, or a stretch where meals became rushed and irregular? Did the last treatment improve the bloating but never really change motility?

Those details are not side notes to me. They are often where the useful information lives.

One of the questions I ask a lot in recurring SIBO cases is not only “What is growing?” but “What stopped moving?” That opens a much more interesting conversation because the small intestine is not just a petri dish waiting for us to kill something. It is a living, moving, neurologically connected part of a much larger system. Motility and digestion matter. The nervous system and hormones, sleep, meal timing, inflammation, constipation, medications, structure and the body’s overall capacity all matter, too.

And no, this is not where I tell you that your SIBO is secretly just stress and you need to meditate harder. Puh-leeease. I would like that particular wellness take returned to sender.

It means that if a problem keeps repeating, I want to understand the conditions around the repetition instead of automatically reaching for Round Four of whatever we did last time.

This is where sequencing can change everything.

Sometimes the antimicrobial phase really is the right next move. Other times I look at the body in front of me and think, Nope. We need to build some scaffolding first. Maybe motility needs help, or digestion is struggling. Maybe elimination is inconsistent, the body is reactive to everything, sleep is terrible, hormones are shifting. It can be all manner of things, singularly and in combination!

That does not mean we do nothing. It means we stop confusing more aggressive with more intelligent.

This is honestly some of my favorite work because I love the case where the pieces look unrelated at first. The symptom that started before the SIBO diagnosis. The supplement that inexplicably helped. The food reaction that does not fit the expected pattern. The weird timing, the lab result everyone glanced past. The treatment that technically “worked,” except the person never really felt well afterward.

I like to say, “Give me that spaghetti.” LOL I want to lay it all out, zoom out far enough to see what is repeating, and then figure out what actually deserves our attention now instead of sending you back into another six months of supplement roulette and Dr. Google.
